Responsibilities

• Acts as a liaison between hospice and the facility to maintain effective communication
• Upholds a positive image to the public through effective planning, programming, advertising, special events, and care management
• Specializes in professional Nursing care to patients in facilities
• Interacts with patients, families, physicians, and other Health team members
• Reviews admission data as defined by policies and procedures
• Assesses patient status and responds in timely manner
• Implements priorities for Nursing action in logical sequence according to patient’s needs
• Formulates a plan of care by the appropriate selection of Nurse Practice standards
• Supervises the provision of services provided by LPNs based on the plan of care and provides feedback on performance as indicated

Qualifications

• Associate degree in Nursing with current license
• One year practice as a Registered Nurse
• Acute Care, Home Health, or Hospice experience preferred
• Knowledge of Hospice Philosophy of Care
