About the Role:
The Clinical Care Navigator I (CCNI) role is to support clients as they enter their journey into Octave, and beyond. This role will contribute to Octave’s mission of expanding access by introducing clients to our innovative practice, and guiding them through the behavioral health landscape. Additionally, CCNI’s will interact as needed with our Provider network, as they field questions and provide day to day support on Care Navigation related tasks.
Ability to work the following schedule (hours/days):
- Tues-Fri 11am-7pm PT; Sat 9-5pm PT
Duties/Responsibilities:
- Support expert client matching with multiple avenues, such as Live Chat, Consults, and other client entry points.
- Conduct scheduled or real-time outreach to understand higher risk clients’ needs, and take action when necessary with the support of clinical staff.
- Support clients along their care journey, from entry into our system, all the way to their first appointment with their provider and beyond via email or live chat.
- Demonstrate a keen operational eye in order to execute on frequently iterated, complex workflows.
- Gain proficiency in our tools and workflows, especially when they evolve as we grow and scale, to successfully navigate our internal operations.
- Assist in collaborative internal workstreams, as needed.
- Collaborate with providers to offer initial clinical insights as applicable and support them with client care transitions and caseload utilization.

Required Skills
- Excellent verbal and written communication and can adapt your approach to multiple groups.
- Detail oriented and impeccable organizational skills are a must.
- Operational experience within a fast-growth healthtech organization is ideal.
- Self-starter, willing to roll up your sleeves and be proactive and solution oriented.
- Nimble and open to frequent change, iterative processes, and operating in a testing culture.
Education & Experience
- Preferred 1-2 years experience working in a Care Navigation/Coordination/Intake capacity, specifically within the mental health or healthcare arena.
- Masters degree in social work, clinical mental health or related area or equivalent combination of education and experience.
- Experience with telephonic crisis intervention.
Physical Requirements
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to frequently communicate with others through virtual meeting applications such as Zoom and Google Meet.
- Must be able to observe and communicate information on company provided laptop.
- Move up to 10 pounds on occasion.
