Job Description

Exciting Opportunity at MSK: The Department of Psychiatry and Behavioral Sciences is seeking a Discharge Coordinator to join our team. As the preeminent group in the psychiatric, psychological, and behavioral sciences domains of oncology, we provide coordinated, comprehensive outpatient psychiatric and psychological services to adult and pediatric cancer patients and their families.

 

The Discharge Coordinator will manage the discharge process for all patients who have been identified as needing community resources or referrals for ongoing care. Within this role, you will participate in the development of tools and resources to continuously identify patients for discharge. You will also measure the successful referral of patients into community settings and maintain a set of active resources and relationships with community providers and resources as needed.

Role Overview:

  • Collaborate with a multidisciplinary team in facilitating a safe discharge plan for patients
  • Act as the key point of contact for patients, assisting in providing appropriate community referral information and acting as a bridge to the next level of care
  • Actively identify and troubleshoot issues that come up during the process (e.g. scheduling appointments, insurance coverage questions, etc.)
  • Adhere to all documentation guidelines established by the department, using applicable databases/reports and the medical record to track and document activity
  • Partner with clinical and administrative leadership in the service on service-level projects and initiatives

 Key Qualifications:

  • Experience working in a psychiatric setting in a hospital or clinic and working with individuals in a case management/care coordination capacity
  • Demonstrated knowledge of mental and behavioral health community resources and providers
  • Proven track record of being proactive and resourceful

 Core Skills:

  • Capable of building strong relationships and working collaboratively to deliver patient-centric care
  • A good decision-maker, with proven success in prioritizing and independently managing work and making timely decisions aligned with individual needs and organizational goals
  • Flexible in your approach and demeanor to align with the shifting demands of evolving circumstances

 

Additional Information:

 

Hours: Monday - Friday, 9 am - 5 pm, hybrid schedule (3 days onsite)

Location: 633 3rd Avenue

 

Pay Range: $60,000 - $93,100 annually
