Description
We’re seeking an individual who is personable, friendly, detail-oriented, and an excellent communicator. Our Care Coordinator collaborates with internal teammates to provide primary health care coordination to individuals in our Sinnissippi service area. Responsibilities include conducting home-based needs assessments and coordinating care for a variety of health needs such as primary care services, durable medical equipment, home health services, meal delivery, dental, vision, etc.
Position essentials:
• Strong organizational and interpersonal skills. Anticipates needs and communicates agency commitment to quality service. Strong decision making and problem-solving skills.
• Detail-oriented. Proficient in managing multiple tasks and managing schedule to ensure timely delivery of care.
• High customer service performance. Dependable, personable, and genuine interest to support clients.
• Strong computer skills. Proficient in Excel and other Microsoft products. Zoom video conference experience a plus.
• Conducts client assessments. Listens, assesses, and identifies the unique needs, interests, and personality of clients and families to create customized client service plans.
• Communicates professionally with health providers and families to ensure that service plans are integrated and supported.
• Compiles client service documents, consent forms, client service plans, and progress notes.
• Provides proactive, positive communication with client/family to facilitate, retain, and ensure coordination of services.
• Collaborates with teammates to schedule initial assessments. Provides follow up, as needed, to current and potential clients. Documents activity in respective tracking log/EMR.
• Provides administrative support for office staff and clinicians as well as any miscellaneous projects.
Position is community based and will require daily travel throughout the counties in our Service Delivery Area.
Job Type: Full-time
Pay: $49,760+ per year
Requirements
Successful candidates will have good customer engagement skills and a "can do" attitude. Experience with care coordination or healthcare systems is preferred.
Position Community Based as services are provided in the community as well as our area offices and will require travel within a 2-3 county area.
Bachelor's degree and/or nursing license required.
