About the role

This position supports a healthier community for older adults by helping raise awareness of the need for direct care workforce positions in NH. This position develops SMART goals for calendar year 2025, establishes action plans in collaboration with workgroup members with actions needed and tasks/persons responsible, with due dates. They will revisit and revise action plans at meetings to include work to be done in between meetings. They will establish meeting agendas and performance metrics with data sources to help the workgroup that they support determine effectiveness in meeting goals.

Duties/Responsibilities

  • Facilitate program meetings.

  • Review and update program action plan.

  • Review program SMART goals, provide update on progress and annual accomplishments.

  • Bachelor’s degree
