Location/Schedule: This position is based at the Kerns building located in NE Portland, OR. The schedule for this position is Monday through Friday, 8:30 a.m. to 5:00 p.m.

Position Description: Under general direction, the Registered Nurse for Forensic Assertive Community Treatment (FACT) coordinates a wide range of supportive and therapeutic behavioral health services for a caseload of adults with severe mental illnesses and addiction issues with criminal justice involvement while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving mental health services, it is required that the FACT Registered Nurse (RN) understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. The Registered Nurse assumes responsibility for treatment decisions in keeping with RN licensure provisions and renders care prescribed by physicians and nurse practitioners. Registered Nurse responsibilities shall not be delegated.

• Provide quality nursing care, including all medical interventions and appropriate documentation. 

• Assist in formulating Nursing Care Plans for all medical/physical conditions. Administer PRN medications based on presenting systems and proper assessment to determine the need for the PRN in collaboration with the LMP. Participate in planning, implementing, evaluating and documenting direct client care activities and medication management, based on sound psychiatric principles. 

• Attend and actively participate in the FACT Treatment Team meetings as scheduled. 

• Act as a liaison with medical staff, pharmacy and laboratory services, and other outside agencies when appropriate to assure continuity of quality client care. 

• Administer psychotropic medications, interpret client response to medication and take appropriate action based on current psycho-pharmacy standards. 

• Collaborate and participate as a team member to aid in establishing medical treatment plans and client care. 

• Interact appropriately with clients, visitors and other community members to support the treatment process, and advise on client rights to treatment, refusal of treatment and involuntary treatment procedures as provided by applicable regulations. 

• Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations.  Maintain accurate and up-to-date documentation as required associated with billing, assessment, treatment, client’s progress and other activities. 

• Attend and actively participate in Nursing meetings and client case conferences. 

• Inform clients about their medications and medical treatments, and provide ongoing support/teaching around medication management for clients and their families.  

• Call the LMP with laboratory results and any other issues that need to be brought to the LMP’s attention, such as clarification of orders, changes in clients’ mental or medical status, or other information shared by the client that would be helpful for the LMP to know in order to further engage a client in the treatment/recovery process.  

• Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.  

• Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for clients and staff.    

• Demonstrate responsibility for safety of consumers, staff and property; is familiar with fire regulations and evacuation procedures. 

• Adhere to mandatory abuse reporting laws and HIPAA requirements.
