Social Services Coordinator - Charter House

To develop, implement, and evaluate programs and services to meet the social service needs of Skilled Care Center residents. To conduct information and referral, recruitment, admissions and discharge functions. A Bachelor's degree from an accredited college or university in Social Services or Social Work. The individual must have prior experience working with a group program or organized recreational activities. Experience working with the geriatric population is preferred.Requires a license by the Minnesota Board of Social Work at minimum a LSW (Licensed Social Worker). Candidates who have recently taken the Minnesota Board of Social Work license test may apply. Candidates who already have a test date scheduled may also apply.
