We seek individuals who are driven to make a difference and embody our motto, “To Care is an Honor.” Join Ennoble Care today!</p></div><p>&nbsp;</p> <h3>Position Summary</h3> <p>The Clinical Liaison serves as the first point of contact for hospitalized patients and their families who may benefit from Ennoble Care’s hospice and palliative care services. This role is responsible for educating patients, families, and healthcare partners about Ennoble Care’s programs, supporting informed decision-making, and facilitating smooth transitions of care. The Clinical Liaison collaborates closely with physicians, interdisciplinary team members, and acute care partners to develop and initiate individualized, goal-aligned care plans.</p> <h3>Key Responsibilities</h3> <ul> <li>Serve as the primary liaison between Ennoble Care and acute care facilities, identifying patients appropriate for hospice or palliative care services.</li> <li>Introduce and educate patients and families on Ennoble Care’s hospice and palliative care philosophy, services, and benefits.</li> <li>Provide emotional support and guidance to patients and families during care transitions.</li> <li>Collaborate with patients, families, physicians, and the interdisciplinary team to develop care plans aligned with patient goals and wishes.</li> <li>Conduct nursing assessments and make clinical judgments based on patient needs and eligibility criteria.</li> <li>Initiate care plans and coordinate discharge planning in collaboration with Ennoble Care’s hospice and palliative care medical consultants.</li> <li>Communicate patient goals, clinical status, and treatment plans effectively with interdisciplinary team members.</li> <li>Promote Ennoble Care’s hospice and palliative care programs within hospitals, skilled nursing facilities, and other referral sources.</li> <li>Build and maintain strong relationships with facility leadership and staff through education, in-service trainings, and community outreach.</li> <li>Rotate to other regions or departments as business needs require.</li> </ul> <h3>Qualifications &amp; Requi
