About the role

Aledade Plus augments practice capacity and capabilities through 'practice extension' solutions to support our business objectives of growing value-based care revenue and contribution margin per practice and per LUM. The work being done by Aledade and Aledade Plus supports our mission to do what's good for patients, doctors and society through lower costs, higher quality, and more equitable care resulting in healthier communities.
We are searching for a Program Coordinator to join the team. The person in this role will support the team in identifying, evaluating, and implementing new programs for the Innovation & Partnerships team. The immediate focus will be supporting scaling our Skilled Nursing Facility (“SNF”) Transitions of Care Program, which works with skilled nursing facilities to provide high-quality, cost-effective care to Aledade patients.

Essential Duties & Responsibilities:

  • Develop and maintain primary program tracker within excel and monday.com (assist post-acute transition specialists in transitioning patient information out of partner platform into program tracker and Aledade app)
  • Perform quantitative analysis to support program operations (including analyzing admissions and discharges at target SNF facilities and supporting tracking SNF engagement KPI’s)
  • Perform research (as needed) and/or analysis to identify, scope, and test opportunities that expand the impact of the SNF program and/or other A+ clinical innovation programs
  • Support post-acute transition specialists in preparing for and facilitating weekly SNF communications and in-person SNF/ Clinic visits 
  • Support practice engagement workstreams by scheduling/ facilitating calls with practices considering whether to participate in the program as needed 
  • Other duties as assigned

Required Skills/Abilities:

  • Proficient working with Microsoft Excel (quantitative analysis) and Monday.com
  • Experience working with cross-functional teams (ideally including clinicians, and data analysts)
  • Ability to work in a fast-paced environment and manage multiple priorities
  • Strong analytical skills/mindset
  • Some travel may be required (initial SNF engagement pitch meetings)

Required Education & Experience:

  • Bachelor’s Degree    
  • 2+ years of professional experience 

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer
