Job Summary
SummaryThis role involves guiding patients through their healthcare journey, addressing concerns, and ensuring that patient needs are met in a compassionate and efficient manner.
Essential Function
-Works closely with patients and a multi-disciplinary care team.
-Conducts needs assessments with patients to identify gaps and educates patients as appropriate.
-Identifying, enrolling, and overseeing patients in various disease state programs, delivering updates on their performance, progress and strategic initiatives in a fast paced environment.
-Maintains accurate and up-to-date patient care records and program metrics.
-Identify trends and opportunities for program improvement.
-Collaborate on initiatives to enhance holistic care delivery, incorporating feedback from patients, families, pharmacies, and care team members.
-Foster and grow partnerships with external organizations and stakeholders to expand the program's reach and impact.
Qualifications
Education
- Bachelor's Degree in a Related Field of Study or equivalent years of experience required
Experience
- Experience in healthcare, customer service, or a related field 0-1 year required or Experience working with patients 0-1 year required
Knowledge, Skills and Abilities
- Strong organizational and project management skills.
- Strong oral and written communication skills
- Excellent interpersonal and communication skills with the ability to connect with a broad spectrum of patient populations.
- Proficiency in data management, reporting and analysis.
- Proficiency with EPIC, Red Cap, and Microsoft Excel is desirable.
- Spanish language proficiency is a plus.
