JF&CS is now seeking a full-time Clinician II Older Adult Field Access.


PURPOSE:

The social worker, Clinician II Access will provide clinical services to agency clients, including working with adults, older adults, caregivers, and adults with disabilities. The Clinician will utilize varied treatment modalities and be willing to provide services in a variety of community locations including but not limited to the Dunwoody office and/or telehealth platforms. This position is focused on increasing services and access to services for Older Adults in the community.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct psycho-social, intake assessments
  • Clinical counseling with older adults, caregivers, adults with disabilities
  • Individual, conjoint, family and group psychotherapy
  • Develop and conduct psycho-educational programs
  • Assist with crisis intervention and emergency assistance (no on-call requirement)
  • Complete documentation and related administrative duties within 36 hours of visit and/or timeframe defined by professional standards
  • Complete a minimum of 15 - 18 encounters or visits/week
  • Complete Credentialing process within first 30 days
  • Work hours based on client demand
  • Travel to home visits as needed
  • Other duties as assigned

Requirements

Licensed Clinical Social Worker with experience as indicated above. Willingness to work with managed care referrals. Willingness to mentor. Ability to work as a member of a team. Able to use Microsoft Word and learn electronic medical record system. Conscientious and reliable. Self-starter and takes initiative. Able to independently maintain caseload. Good attention to detail. Meets deadlines.
