The Clinical Partnership Manager is responsible for managing the patient relations function of a healthcare organization to deliver patient liaison services and patient-centric care. Develops materials and optimizes processes to provide information and resources to patients and families. Coaches staff on best practices for responding to patient and family questions, concerns, and issues and to resolve escalated problems. Collaborates with multi-disciplinary teams and senior leadership to make recommendations for improvements to the patient experience to Executive Leadership. Ensures that all applicable regulations and patient information privacy policies are implemented and followed.

Responsibilities
• Aide in the evaluation of potential new clinical partnerships via due diligence, relationship building, and identifying the highest-performing partners in the healthcare space.
• Support key clinical initiatives with third-party vendors and CMS, as well as collaborate with various internal stakeholders to implement and execute clinical partnerships with external organizations.
• Oversee the day-to-day operational aspects of large-scale, complex clinical programs from partnership implementation, execution, performance measurement, and program completion or renewal.
• Support all aspects of clinical partnership that involve patient care, healthcare-provider interaction, internal business partners, and program performance throughout the company's geographies and specialty areas.
• Apply clinical acumen and business principles to monitor Millennium/partnership clinical and financial performance.
• Deliver insightful, data-driven recommendations to management.
• Update existing documents and manage internal databases accordingly.
• Build presentations for senior management and the Board of Directors to assist with strategic decision-making.
• Work with the Executive Leadership Team to establish a vision for transformative initiatives.
• Support and influence clinical partnerships through high-quality financial modeling & analysis.
• Provide the Strategic outlook of the clinical partnership, and effectively communicate financial aspects to Senior Stakeholders with recommendations
• Utilize the Information & Innovation team to leverage technology for initiatives and process improvements.
• Collaborate with the Operations and Population Health teams to craft implementation and on-the-ground plan execution plans.
• Collaborate with the Learning and Development team, and Internal Communication to build relevant content, create internal communication, and assist in change management.
• Demonstrate excellent guest service to internal team members and patients.
• Perform other related duties as assigned.

Qualifications
• 3+ years of experience in managed care and/or physician group industry.
• 3+ years of experience with population health management strategies and implementation, in primary care, and/or value-based care is preferred.
• 2+ years of experience in Project Management.
• Ability to translate results into crisp presentations to senior management; fluency in PowerPoint and Excel is required
• Possess sound business judgment and work with a sense of urgency
• Strong communication skills, ability to present findings and recommendations to a wide range of constituents verbally and/or in presentation formats
• Ability to apply business principles to clinical settings and develop strategies to answer questions that are complex and ambiguous
• Proficiency in interpreting financial models
• Comfort working collaboratively and in a large corporate matrix
• Ability to work independently in a fast-paced, cross-functional environment.
