Responsible for providing general direct and indirect patient education in the home setting under the direction of the COPD Program Director. Responsible for qualifying and enrolling patients into the COPD Disease Management Program. The Certified Medical Assistant (CMA) serves as a key support resource for the COPD Disease Management Program by identifying, qualifying, enrolling, and tracking eligible patients referred to the program. Working collaboratively with the COPD Educator, physicians, care managers, hospital discharge planners, and other healthcare professionals, the CMA ensures timely patient outreach, accurate documentation, efficient scheduling, and ongoing program coordination to improve patient engagement, reduce COPD-related readmissions, and support positive health outcomes.
- Review incoming referrals and hospital discharge reports to identify patients who may qualify for the COPD Disease Management Program.
- Verify eligibility criteria according to established program guidelines.
- Conduct initial patient outreach via telephone or electronic communication to explain program benefits and determine interest in participation.
- Obtain and document patient consent for enrollment.
- Collect demographic, insurance, and clinical information required for program participation.
- Coordinate referral processing and ensure all required documentation is received.
- Maintain tracking logs for referrals, enrollment status, patient engagement, and program completion.
Enter and maintain accurate patient information in EPIC, Bonafide, and other program databases.
- Generate reports related to referral volume, enrollment rates, patient participation, and program outcomes.
- Track patients requiring follow-up outreach and coordinate re-engagement efforts.
- Ensure documentation complies with organizational policies, regulatory requirements, and privacy standards.
Perform additional duties as assigned to support program operations.
- High School Diploma or equivalent.
- **National Certification CCMA from: NHA, AAMA, NAHP, ARMA, AMT, AMCA, or NCCT.
