JOB SUMMARY
The RN Program Coordinator provides comprehensive care coordination for patients with complex conditions across the continuum of care. This role serves as a key liaison among providers, patients, families, laboratories, pharmacies, and external agencies to ensure timely diagnosis, treatment, and long-term management. The coordinator also supports clinical operations, research initiatives, and patient advocacy within a multidisciplinary and academic care environment.

EXPERIENCE AND EDUCATION

Required
  • Education
    Bachelor's Degree in Nursing
  • Experience
    3 years of nursing experience
    Additional experience may be considered in lieu of education requirements
  • Licenses and Certifications
    (RN) REGISTERED NURSE Licensure by Board of Nurse Examiners for the State of Texas Upon Hire
    (BLS) BASIC LIFE SUPPORT Course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire
Preferred
  • Experience
    Experience in complex clinical conditions, rare disease, or specialty care
    Prior care coordination or program coordination experience

JOB DUTIES
  • Clinical Coordination & Care Management:
  • Coordinates intake, evaluation, and longitudinal care for patients with complex conditions.
  • Serves as the primary nursing liaison among multidisciplinary teams and external providers.
  • Reviews referrals and ensures appropriate documentation and follow-up.
  • Coordinates clinical testing, including prior authorizations, specimen collection, lab communication, and result tracking.
  • Manages patient communication through calls, MyChart messages, and nurse visits.
  • Monitors surveillance labs, communicates results to patients and providers, and maintains patient tracking databases.
  • Facilitates transitions of care, including pediatric-to-adult clinic transitions and coordination with inpatient and outpatient services.
  • Coordinates specialty clinic visits and multidisciplinary care plans.
  • Obtains and integrates external medical records into the pat
