Millennium Physician Group is seeking to hire a full-time Patient Care Coordinator for our House Call Program. This is a growing practice and is currently searching for a dependable, out-going individual who enjoys working with patients. The Patient Coordinator is the face of the company and plays a critical role in the quality of service provided to our elderly patients.
The Patient Care Coordinator supports providers in all aspects of patient care. S/he is responsible for collection, communication and documentation of accurate patient information. The Patient Care Coordinator performs phone triage of patient questions/concerns using protocols to determine the level of physician and nurse response required. Additional responsibilities include download and review incoming test reports, maintaining the pharmacy calls, coordinating requests for prescription refills per protocol, scheduling outpatient procedures, referral appointments, outpatient testing and other diagnostic appointments as directed. The Patient Care Coordinator also processes patient letters, forms and other documents as needed by the patients. Compensation is competitive plus a full benefits package including 401(k) with match and 3 weeks of PTO!
Patient Care Coordinator Qualifications and Skills
- Caring and compassionate personality

- Ability to multitask and work under pressure
- Excellent communication skills
- Prior Medical Office experience is preferred
- Assist with referrals/progress notes//medical records/prior authorizations
- Assist MDs and NPs
