JOB SUMMARY
Ardent is seeking a Physical Therapist (PT) to cover Fresno County. The PT is responsible for providing physical therapy services to patients according to a written physician’s plan of care.
DUTIES AND RESPSONSIBILITY
- Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
- Evaluate home environment.
- Assess for muscle strength, mobility, gait, ROM—potential for rehab.
- Direct physical therapy treatment.
- Instruct patients and families/caregivers in the use and care of therapeutic appliances.
- Determine priority needs for physical therapy.
- Report to physician patient’s reaction to treatment or changes in condition.
- Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
- Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences.
- Train patient in the use of prosthetic device.
- Identify patient and family/caregiver needs for other home health services.
- Prepare and submit clinical and progress summaries based on the attainment of goals.
- Participate in discharge planning for patient.
- Prepare and submit a clinical progress summary based on the attainment of goals.
- Provide physical therapy consultation to home families/caregivers.
- Provide in-service education programs for nursing organization personnel.
- Participate in peer consultation process.
- Supervise Physical Therapy Assistants according to organization policy and state regulations.
JOB REQUIREMENTS
- CA Physical Therapy License
- Two (2)+ Years of physical therapy experience, home health experience preferred
- Demonstrate good verbal and written communication and organization skills
- Possess and maintain current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
PAY RANGE: $55.00 - $65.00 per hour
