Nurse Practitioner or Physician Assistant – Heart FailureUniversity of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant to join our cardiology practice in our East Region including Maplewood and Woodwinds. The Position will provide patients with expert patient care and coordination of services. They will assist the practice to improve patient satisfaction and enhance the quality of care provided to patients through increased access and coordinated services.In This Role, You Will:

• Join a multidisciplinary team of 22 Cardiologists (1 HF focused) and 12 Advanced Practice Providers (2 HF focused)
• The PA or NP provides and coordinates care for adults with cardiac diseases throughout the course of evaluation and treatment.
• Participates primarily in outpatient management for adults with heart failure of various etiologies.
• Provide patient and family education as it relates to heart failure and general cardiology.
• Provide clinic coverage in heart failure and general cardiology
• This individual will show commitment to exemplary patient care.
• Manages patient care through appropriate use of available resources.
• Demonstrates ability to deliver care or service adjusting approaches to reflect developmental level of population served.
• Utilize a fully integrated Electronic Medical Record (EPIC)

Schedule:

• 1.0 FTE
• Position is primarily clinic based, with possibility of expanding into the inpatient setting in the future.

We’re Looking For a PA or NP Who Is:

• A graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
• Eligible or is certified through NCCPA, ANCC or AANP.
• Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN APRN license or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP’s).
• Able to meet threshold criteria/qualifications for credentialing and privileges.
