The Vice President of Supportive Housing is responsible for furthering the mission of CCH by providing executive leadership, vision and guidance of multi-disciplinary teams providing supportive housing and services to homeless and at-risk families and individuals throughout CCH. The position will work with the Integrated Health and Supportive Housing Executive Team to provide oversight of current departments, develop new initiatives, improve clinical and program outcomes, and strengthen program integration and infrastructure across CCH. The position will ensure that all CCH housing programs provide consistent, quality and integrated housing and services, and will guide the development and expansion of existing supportive housing services throughout CCH. The position will also ensure integration with and access to health care, community services, and other resources provided by CCH.

Essential Job Functions
- Works collaboratively with Integrated Health and Supportive Housing Executive Team to lead, develop, manage, and supervise integrated housing services provided by all areas under their direction in accordance with the applicable policies and procedures established by the Board of Directors, program funders and management staff.
- Hires, trains and supervises the Directors of Housing Support Services, the Director of Family Support Services and the Grants and Data Reporting Manager.
- Serves as an active member of the Integrated Health, Supportive Housing and Services Executive Team to promote and ensure integration with and access to health care, supportive housing, and community services provided by CCH.
- Works closely with the Vice President of Property Management and the Vice President of Housing Assistance to streamline the housing placement and housing retention processes, identify unmet needs, and enhances communication and collaboration between programs, housing assistance and property management staff.
- Oversees the development of robust integrated health services and client programming at all CCH Housing locations to include identification of service gaps, creation and implementation of shared integrated care plans, culturally relevant and inclusive community groups and activities, level of care programming and other promising and evidenced-based practices in the provision of supportive housing.
- Serves a clinical consultant on complex client care and housing related concerns, offering support and guidance to staff to achieve positive outcomes.
