Job Summary:

Under the direction and supervision of the Director of Nursing, maintains resident’s medical records in accordance with facility policies and regulations. Under the supervision of the Administrator, the transportation driver will provide transportation for residents in the center van or accompany them with a third-party provider to their medical appointments and activities. Nurse Aide certification is required as this individual may be required to perform non-professional, direct resident care duties. This position will assist in maintaining a positive physical and psychosocial environment for the residents. This includes assisting residents with activities of daily living, recreation programs, and eating, as needed. When not transporting residents to appointments or activities other hours can be spent working in the center performing the duties of a Certified Nursing Assistant.


Job Description:

  • Sets up resident’s medical records; collects information from nursing staff, physicians, and other sources and updates records; monitors and audits records for completeness and accuracy in accordance with facility policy and regulations and provides information to business office.
  • Participates in completion of MDS, Utilization Review meetings and related functions.
  • Processes transfer or discharge records and maintains appropriate records on former residents.
  • Assists in Nursing Department activities to include administrative support, assisting residents, families, and visitors, and running errands to include travel to hospitals and doctor’s offices.
  • Attends orientation and in-service training as required by the state.
  • Understand, comply with, and promote all rules regarding residents’ rights.
  • Promote positive relationships with residents, visitors, and government regulators, to include presenting a professional appearance and attitude.
  • Attends and participates in training, educational activities and staff meetings.
  • Fills in as Nurse (if licensed) or CNA or other nursing position for which qualified as needed.
  • Transporting resident's to and from appointments.
  • Other duties as assigned.

Requirements

  • Certified Nursing Assistant (CNA) in good standing with the state or must within four (4) months of employment have completed state-required training and a competency evaluation program.
  • Clean driving record.
  • Ability to relate positively to residents and families and work cooperatively with other employees.
  • Capable of maintaining regular attendance.
  • Meet all local health regulations, and successfully complete a post-offer health assessment.
  • Physical and sensory requirements (with or without the aid of mechanical devices): walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing and pulling, ability to hear and respond to pages; ability to read and write; ability to communicate both verbally and in writing with residents, families, personnel, vendors and consultants; ability to remain calm under stress; ability to distinguish smells, tastes and temperatures.
