Family Medicine Physician

 

Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations. CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care. 

 

We are seeking a mission-driven Physician to join our team at the VNACJ Community Health Center at Keyport. In this role, you will provide holistic primary care to individuals and families of all ages, ensuring access to essential health services through collaborative teamwork. You will work alongside a dedicated team to drive clinical excellence, contribute to quality improvement initiatives, and support community outreach efforts. Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their  job with a focus on positive interpersonal relationship with co-workers
  • Provides direct family and patient centered medical services and consultation to primary care patients and families at the sites within established productivity guidelines
  • Maintains complete patient records using the Electronic Medical record and complies with all reporting mechanisms
  • Ensures compliance with the New Jersey Department of Health and Senior Services Ambulatory Care Regulations for Licensure as well as requirements of other licensing and accrediting bodies, health maintenance organizations and other insurance carriers
  • Collaborates in team based care in the Patient Centered Medical Home model
  • Collaborates in Peer Review and other Quality Improvement activities
  • Collaborates in the provision of in-service education for staff
  • Adheres to CHC infection control and safety policies, and guidelines of regulatory agencies
  • Participates in community outreach programs regarding CHC services and programs
  • Supervises staff in conjunction with the organization's managerial and leadership practices
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  • Licensed as a physician in New Jersey with education and experience in primary care settings
  • FQHC experience preferred
  • Board certified in family practice
  • Active CDS license
  • Car and valid NJ driver’s license

Working Conditions/Physical Demand: Working indoors.  Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing, walking, lifting 40 pounds, pushing, reaching, pulling
