Description

WeCare Centers at Jameson Care Center is actively hiring an experienced RNAC, LPNAC, or MDS Coordinator for our community in New Castle, PA — now offering a $2,500 Sign-On Bonus for qualified candidates.

We are seeking a detail-oriented nurse with strong MDS, RAI, and long-term care reimbursement experience to support accurate assessments, care planning, and regulatory compliance in our skilled nursing facility.

Final title and compensation determined by licensure and experience.

Key Responsibilities

• Coordinate and complete the RAI/MDS process
• Ensure accurate and timely MDS submissions and documentation
• Conduct Care Area Assessments (CAAs) and develop care plans
• Monitor Medicare & Medicaid compliance and reimbursement accuracy
• Collaborate with interdisciplinary team on resident care planning
• Maintain regulatory compliance and survey readiness
• Support quality measures and quality assurance initiatives
• Educate staff on MDS and care planning processes
• Additional duties as assigned by leadership

Skills, Knowledge and Expertise

• Active Pennsylvania RN or LPN license required
• MDS/RAI experience in a skilled nursing facility strongly preferred
• RAC-CT certification preferred
• Knowledge of Medicare, Medicaid, and PDPM
• Strong clinical assessment and documentation skills
• Detail-oriented with strong organizational skills
• Ability to work independently and as part of a team
