The Patient Navigator will work with physicians and staff to ascertain the appropriate services the patient needs; schedule the patient appointments with multiple departments; and follow up with the patient to assure compliance. The Patient Navigator supports delivery of the Jefferson Experience by placing emphasis on the prompt and courteous treatment of all patients and visitors.Job Description
Essential Functions:
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- “Point-Person” for all activities for the surgeon(s) team and coordinates all communications between the patients, clinical team and office staff.
- Monitors and performs clinic prep, works with patients to schedule testing, schedules and coordinates surgeries and follows patients through from their NPV to surgery to post operative care.
- Assist the surgery scheduler on the team and communicate/facilitate case changes when they occur.
- Greets and interacts with patients/visitors in a professional manner, on phone or in person.
- Adheres to HIPAA privacy and security requirements. Ensures compliance with all applicable federal, state, and local regulatory standards.
- Other duties as requested or assigned.
Education and Experience
High School diploma or GED required.
Bachelor’s Degree in psychology, health administration or other relate fields from an accredited college/university preferred.
3-5 years of experience in a medical office required.
