San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a Full Time onsite position. The typical schedule for this role is Monday - Friday, 8:00AM - 4:30PM.
Job Summary: Manages and coordinates all aspects of St. Paul’s PACE supportive housing program including program operations oversight, acts as the liaison for housing
services in the community to ensure ongoing and successful living for the PACE participants.
Qualifications and Requirements:
Education: Bachelor’s degree an accredited college or university. BSW,
psychology or equivalent relevant degree.
Experience:
• A minimum of one year’s experience in working with the frail or elderly
population required
• Recommend at least one year experience working directly with homeless
housing services organizations, Seniors 55 plus, and knowledge of the Veterans community.
• Prefer four years of related experience and/or training or an equivalent combination of education and experience.
• Must possess strong computer skills and knowledge in the use of software application programs in word processing and spreadsheets. This is to include at a minimum such programs as Word, Excel, and PowerPoint.
• Must be willing to learn or have experience with the Coordinated Entry System (CES) Service Point and Shelter Point IT systems.
• Excellent communication skills to impart the value of all St. Paul’s services.
• Must be able to attend after hours and weekend events to secure support for the program including fundraising activities, engaging volunteers, and creating community awareness of the program.
• Bilingual in Spanish is an asset

• Must have strong interpersonal skills to effectively communicate with and
relate to Community members, Board members, participants, residents, donors, diverse Volunteers and staff.
• Must possess excellent verbal and written communication skills. Must be able to effectively create and write reports and correspondence.
• Must possess strong organizational skills to successfully plan for and meet deadlines, schedules and goals.
• Must be self motivated and not easily distracted from the task of serving this vulnerable population.
Duties and Responsibilities:
Program Management and Oversight
• Manages St. Paul’s PACE supportive housing program and staff.
• Designs and develops a comprehensive program for identifying, accessing, assessing, and coordinating PACE participants who are homeless or near homeless into permanent housing, with a strong emphasis on Veterans.
• Locates housing in San Diego County for those not assigned to PACE identified units.
• Represents St. Paul’s PACE at community meetings and provides outreach for the Housing program.
• Develops and keeps current all policies and procedures and guidelines for permanent supportive housing services for St. Paul’s PACE and all other St. Paul’s communities.
• Develops a process for accessing referrals, assessing housing needs, connecting with housing agencies, application processing, move in process, collaboration with PACE and other supportive services, and monitoring program success and quality.
• Develops, delivers tabulates and reports program participant satisfaction with their housing situation.
• Tracks outcomes of housing on the participants health and social engagement.
• Maintains computer records of all community outreach meetings & events, potential participant interactions, volunteer interactions, applicant tracking, supportive housing resident interactions, housing program meetings, and other pertinent daily activities.
• Coordinates the preparation, invitation, and follow-up for meetings and special activities.
• Coordinates recruitment, on-boarding, initial and on-going training, of volunteers for supportive housing related projects. Utilizes volunteers to support the programs.
• Coordinates with the Volunteer Coordinator and the Foundation to ensure the housing program is ready for new participants to move in.
• Represents St. Paul’s PACE at regular meetings with Housing Owner/Property Management as requested in the Memorandum of Understanding (MOU) and
additionally as needed.
• Negotiates and executes conditions of contracts with partner agencies independently.
• Maintains at least regular monthly contact with the Owner’s property manager via
