Position Description:
 

The Certified Pharmacy Medication History Technician is responsible for obtaining an accurate medication history for patient's being admitted to TGMC. The medication history will include prescribed medications, over-the-counter medications and dietary and herbal supplements. Routine functions also include instructing patients/family members to maintain updated medication lists, communicating pertinent findings to appropriate care team members and identifying and facilitating correction of medication discrepancies. All job duties will be performed under the direct supervision of a licensed pharmacist.


Qualifications:
1. Certified and registered as a Pharmacy Technician with the Louisiana Board of Pharmacy required.
2. Minimum 2 years of retail experience preferred, but not required.
3. Experience with writing technical reports is desirable.
