🩹 What You’ll Do

📝 Conduct comprehensive in-home assessments and reassessments
📋 Create & update individualized care plans
💊 Provide medication administration and skilled nursing interventions
📈 Monitor & document client health changes
💬 Communicate with clients, caregivers, and healthcare teams
🔗 Coordinate referrals to healthcare or community resources
🕒 Maintain accurate documentation per state and agency regulations
🎓 Participate in caregiver training and in-service programs

🎯 What You Bring

🎓 Current Maryland RN license (or multi-state compact license)
❤️ CPR/First Aid Certification
🏠 Experience in Home Health, Hospice, or Geriatric Nursing preferred
🧠 Strong assessment & clinical decision-making skills
🚗 Reliable transportation for home visits
🕰️ Willingness to work occasional evenings/weekends
🧴 Knowledge of infection control and patient safety procedures
