Job Description:
The RN Navigator, or Nurse Navigator, is a registered nurse who acts as a patient advocate and care coordinator, guiding patients through the healthcare system as it relates to Intermountain Health’s front range cardiovascular clinics. The RN Navigator provides education, support, and facilitates communication between different healthcare providers to ensure timely and appropriate care. This position is a part of the cardiovascular support team which includes handling urgent patient issues sent from the Contact Center team, reviewing referral requests from primary care offices, and addressing critical hospital referrals. Responsibilities also involve analysis of new patient medical records prior to their first cardiovascular visit to screen for pre-testing needs. Additional scope of responsibility:
Handles and directs calls from patients that have an urgent cardiology concern; calls may vary from routine to emergent triage circumstances. Follows recommended triage protocol formats and directs patients to next care steps. Demonstrates professionalism, efficiency, and sound clinical judgement while interacting with patients and colleagues.
Provides clinical guidance to both the Intake and Authorization Teams concerning incoming referrals from primary care offices or other referring providers. Guides decisions regarding a patient’s plan of care and the appropriate cardiology subspecialty to which they are directed if necessary. In addition to patient outreach, the RN Navigator often will contact the referring provider to obtain additional information or clarification regarding patient condition. May also be involved in arranging peer to peer consultations.
Reviews incoming patient referrals from Intermountain hospital emergency departments and inpatient providers advising a patient’s next care steps. Contacts these patients following
their discharge and promotes the Intermountain Health cardiovascular clinics for their follow- up care.
Complex scheduling of procedural and diagnostic testing appointments for multiple clinics and providers based on diagnosis and provider/clinic preferences; must be able to follow
pre-determined protocols for scheduling and provide pretest instructions to the patient.
Analysis of new patient chart or medical record information following protocols to assess if pre-testing is necessary to assist the cardiologist in their initial diagnosis and treatment plan.
Requires knowledge of Excel spreadsheets and their manipulation, electronic medical record navigation (EPIC), organization skills, and the ability to quickly review medical information
and/or other patient information.
