The Southeast Health District is seeking a highly qualified candidate to fill the full-time, salaried position of PH Registered Nurse with the DPH Home Visiting program.

Examples of duties
Under general supervision, will provide professional nursing care in accordance with nurse protocols and programmatic policies and procedures. Provides outreach, support and education to the target population and their surrounding community in order to reduce disparities in birth outcomes. Provides community health and case management services to high-risk perinatal women, infants, and children primarily through home visitation. Provides patient education and refers to agencies that can assist in meeting social and/or health needs. Records pertinent observations and response to treatment.Qualifications

Southeast Health District accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. SEHD will contact educational institutions to verify degree, diploma, licensure, etc.
