Job Description Summary:
According to department policies and nursing care standards, provides care that is appropriate to the age of the patients served. This includes administering medications, giving treatments, observing patients and recording and reporting pertinent information under the supervision of a registered nurse.Responsibilities And Duties:
1. Maintains a safe and therapeutic patient care environment. 2. Demonstrates competence in providing care and services to patients based on the patients age specific and/or functional needs. 3. All employees shall abide by the Health Insurance Portability & Accountability Act HIPA a regulations, which help protect the privacy, security, & confidentiality of patient health information. 4. Responsible for ensuring that personal performance reflects the Mission, Vision and Service Pride Standards. 5. Assists Registered Nurse in admitting patients to unit by orienting patients and family to room, introducing unit personnel, collects patient data, and explaining privileges and restrictions. Takes and records patients' vital signs e. g. , blood pressure, pulse, temperature, respiration, etc. upon admission and on regular basis according to physician orders and nursing protocol. 6. Administers medications/fluids to patients orally, by subcutaneous or intramuscularly injections, or IV according to nursing policy and procedure, and as prescribed by physician. Notifies appropriate nursing personnel of contraindications and withholds further medications until otherwise directed. 7. Observes patients for color and condition of skin; response to medication, treatments and diet; changes in neuro status; symptoms of infection; unusual symptoms; adverse reactions; emesis, drainage, discharges or bleeding; behavioral changes; and/or post-operative changes. Reports significant observations to Registered Nurse and documents in charts. 8. Provides direct technical nursing care to patients by performing a variety of semi-complex treatments such as changing dressings, cleansing and/or irrigating wounds and incisions, performing bladder catheterizations and irrigations, perineal care, tong/pin site care, care of feeding tubes and limited IV therapy if qualified. 9. May assist healthcare team members with more complex patient care and treatment procedures such as deep wound irrigations, airway management; burn care, isolation procedures, and central line dressing changes. 10. Assists Registered Nurse with patient discharge process by reinforcing instructions self-care/home care techniques required to maintain activities of daily living or continued rehabilitation and healing. Documents patient response and compliance with appropriate teaching protocols. 1 1. Assists patients with activities of daily living including personal hygiene and grooming, bathing, eating, changing clothing, etc. Provides patients with bedpans or urinals, and empties and cleans same; or assists patients to commode or bathroom. 12. Monitors, measures, and records patients' intake and output according to established nursing protocol and physician orders. 13. Assists patients with ambulating or exercising by walking with patients. Follows established safety standards to ensure patient safety. Repositions patients in chairs and beds. 14. Collects patient specimens e. g. , urine, stool, sputum, etc. , labels and sends to laboratory for analysis and documents appropriately. 15. Attends department and Hospital meetings. 16. Consults with the Registered Nurse to identify nursing care priorities and acts on that information to organize patient care. Organizes care according to workload requirements and time allotted. Sets and adjusts priorities according to patient care needs. 17. Provides emotional support to patients and families. 18. Provides the patient/family with health care information as appropriate following the plan of care. Uses standard teaching protocols and available resources. 19. Provides direction and guidance to other HCT members NA, UC, PCA, etc. in the delivery of patient care. 20. Performs other duties as assigned, some of which may be essential to the job.
