This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Lexington County Community Mental Health Center, 301 Palmetto Park Blvd, Lexington, SC 29072.

Responsibilities for MHP- Assertive Community Treatment-included but are not limited to:
Under general supervision, serves on the Assertive Community Treatment (ACT) team providing evidence-based services (safe, effective, and measurable) in an intensive non-residential treatment and rehabilitative services to promote and support the recovery of persons diagnosed with a serious mental health condition. Guides patients in development of skills or strategies toward achieving and maintaining good mental health. 

Responsibilities for the Mental Health Professional:

1. Provides direct services to ACT patients guiding them in the development of skills/strategies towards achieving and maintaining good mental health and facilitating training of independent living skills.  Utilizes assertive engagement to engage beneficiaries in service. 
2. Assists patients with access to community services, resources and agencies and establishing support networks in the community. Utilizes the patient's natural supports. 
3. Completes screenings and assessments for referrals and assists with development and revisions of instructional continuity plans (ICP).
4. Provides family psychoeducation and works with beneficiaries' natural supports.
5. Leads the person-centered planning process for patients of the ACT program.
6. Participates in clinical supervision, treatment team meetings, pursues professional development, and demonstrates effective use of social and interpersonal knowledge and skills and demonstrates an understanding and appreciation of individual and group differences and similarities. Effectively utilizes and maintains certification of evidence-based practices. Attends treatment teams meetings daily and provides consultation regarding complex cases as appropriate. 
7. Completes documentation according to QA/SCBHDD standards, complies with Corporate Compliance Policy and Risk Management Plan, and ensures timely completion of prior authorizations.
8. Completes all annual required trainings to include Safety, MAPS, Cultural Affairs, and clinical trainings. Demonstrates adherence to CARF standards, and SCBHDD and Center policies and procedures.

Examples of duties

Minimum and Additional Requirements:

•  A master’s degree in guidance, counseling, the social sciences or a related field

Internal Requirements:

• Licensed as an LISW-CP, LMFT, or LPC in the state of South Carolina.  
• A master's or doctoral degree from a program that is primarily psychological in nature from an accredited university
• One year of experience working with adults with serious mental illness
• Position requires flexibility of work hours and participation in afterhours crisis response

 Additional Requirements:

• Ability to lift 5lbs. and operate a state vehicle. Position is considered essential; and may be called back during crises/disasters/emergencies. Position will be part of the Assertive Community Treatment Team. Requires flexible schedule. Services are delivered in the community and patients' homes.

Qualifications

PREFERRED QUALIFICATIONS:

• Bilingual abilities in English and Spanish (or another language) are a plus.
