Hospice RN

• The Hospice RN assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families.

Requirements:

• Current New York State Registered Nurse License/Permit
• Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice.
• At least one (1) year of community hospice or home health experience is preferred.
• Hospice experience is preferred.
• New York State Driver's License, preferred.

Shift/Status:

• Open Availability in Manhattan, Bronx, Westchester, Queens, Brooklyn, and Nassau County.
• Full-time, Part-time and per-diem opportunities available.

Hospice Admission RN

• The Admission RN is responsible for responding to and visiting patients who have been referred for admission to Calvary @ Home. The Admissions Nurse informs the patients, caregivers and family members of hospice services available with the organization and asses the patient for admission to services.

Requirements:

• Current New York State Registered Nurse License/Permit

Requirements

Hospice Social Worker

• The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients and families.

Requirements:

• MSW from an accredited school of social work and one year related experience.
• LMSW or LCSW eligible.
• Minimum one (1) year bereavement counseling experience is preferred.
• At least one year of Hospice or Home Care experience preferred
• New York State driver's license.

Shift/Status:

• Full-time availability in Bronx and Westchester.
