Position: Full-Time Licensed Practical Nurse (LPN)Location: Phoenix, AZ (Territory Coverage)

Qualifications:- Graduate of an accredited practical nurse or vocational nursing program- Minimum one (1) year of experience, preferably in home care or med/surg- Current licensure in the state and CPR/BLS certification- Strong communication and problem-solving skills- Ability to meet physical demands of the job- Valid driver's license and reliable, insured vehicle
Responsibilities:- Implement personalized care plans under clinician supervision- Maintain accurate and timely documentation- Provide patient and family education on diagnoses and care plans- Coordinate with interdisciplinary team members for comprehensive care
