Description

Full-time: The Patient Advocate delivers exceptional customer service in our Platinum program which provides preventive, primary and acute care to members. The Platinum team is available 24/7 for medical assistance. Duties include but are not limited to scheduling appointments for internal and external services, documenting in the EMR, assisting physicians with medical services as directed, and assisting the program with administrative duties as assigned by the Executive Director of the program. This position requires the employee to be available, after hours and on weekends, to assist our members.


Schedule: Monday - Friday 8:00 a.m. - 4:00 p.m. plus on-call for Platinum patients and physicians


Patient Advocate Essential Duties and Responsibilities

· Responds to patients’ health care needs and assists the patient and physicians during and after office hours.

· Answers phone calls, takes messages, and schedules patients for services.

· Coordinates care for internal or external medical services and follow up care, including Rx refills.

· Assists physician with appointments by taking vital signs, EKG’s, and other primary care procedures as needed.

· Obtains, retrieves and sends medical records of patients.

· Assists with hospital/ER admittance.

· Documents all patient interactions, tracks clinic services needed and rendered.

· Visits on occasion, patients in the hospital or other facilities.

· Promotes the program and renews memberships which include general contracts and invoice processing.

· Completes duties on site with consistent, regular, and reliable attendance.

· Accomplishes other duties as assigned.


Requirements

Required Skills/Abilities

· Excellent verbal and written communication skills.

· Extensive experience with charting, medical terminology and EMR functions.

· Exceptional interpersonal and customer service skills.

· Strong organizational, time management and attention to detail skills.

· The ability to multi-task is essential.

· Possess problem-solving skills, be self-motivated, and strive for professionalism.

· Ability to work well in a fast-paced and team environment.

· Good computer skills with knowledge of Microsoft applications.


Education and/or Experience

· MA/LVN, or a Bachelor’s degree with clinical experience. Or minimum of 5+ years of clinical experience with patients.

· Ability to work 24/7 on call required.


Physical Requirements

· Must be able to lift up to and occasionally move up to 25 pounds.

· Prolonged periods of walking, standing, and/or sitting at a desk.
