About the role

Medical Case Management (MCM) ensures that people with HIV access, receive, and stay in primary medical care. MCM assess the primary and immediate needs of people with HIV, coordinate referrals, and follow-up with critical core medical and support services to ensure people with HIV remain in medical care. The services that are provided are in alignment with the National HIV/AIDS Strategy and focus on entry into care, retention in care and viral load suppression. Key activities include initial assessment of service needs, development of an individualized plan of care, coordination of services required to implement the plan, and monitoring of care plan.  MCM includes face-to-face appointments in office, via home visit, or at provider offices, phone contact, and any other form of communication.  Supports the development and execution of community outreach strategies, initiatives and events that align with IU Health community outreach priorities, in meeting the identified needs of the communities. Provides assistance to team member volunteer programs. Provides administrative coordination for the system team in areas such as meeting management, budget coordination, and ad hoc project support. Supports the assigned Manager and team in successfully executing initiatives and events.

  • Bachelor's degree or equivalent years of experience required.
  • 0-3 Years of Experience.
  • Ability to work in a complex business environment.
  • Ability to manage multiple projects and deadlines, background managing program/event logistics.
  • Experience in managing program/event logistics preferred.
