Job Title - LPN SUPERVISOR

Reports to - Director-Residential Services with clinical supervision provided by the Nurse Manager

Job Classification – Exempt

POSITION PURPOSE AND GENERAL SUMMARY

While respecting the rights of the people we support, the Nurse Supervisor will be responsible for the acquisition and provision of health care within established nursing objectives and standards, adhering to current nursing law, regulatory requirements, and to the Skills Residential Programs Nursing Roles Policy. This responsibility includes being involved in all high risk, complex health care situations to include hospitalizations, investigations, and establishing individualized medical protocols. The Nurse Supervisor will provide ongoing health care monitoring to those we support in the Residential, respite care and Lifesharing programs with consultation/training as requested from the day programs. The Nurse Supervisor will be responsible for the direct supervision of the nurses in the Residential program.

LPN SUPERVISOR ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Maintain a high level of accountability and dependability to include attendance at work and punctuality providing direct care when needed.
2. Complete and maintain certification as a Medication Administration trainer.
3. Monitor the electronic medication records for quality assurance
4. Complete and maintain certifications as a Health Risk Screening Tool (HRST) rater and reviewer
5. Will review all Health Risk Screening Tools (HRST) for the region in the Community Residential Program including Lifesharing within 14 days of the completion of the initial screening.
6. Complete and maintain certification as a Certified Investigator.
7. Participate in administrative reviews for incidents involving medical concerns.
8. Assist in hiring, orientation and ongoing training of the nurses.
9. Monitors the acquisition, delivery and quality of health care within the Community Residential and Lifesharing Programs by:
10. maintaining an on-call system of support for the Residential program
11. providing routine supervision to the nurses
12. systematically reviewing people’s records to identify trends for systemic needs and determine effectiveness of protocols
13. facilitating the monthly nurses’ meeting to ensure consistency with established policies and procedures
14. Ensuring compliance with evaluation and annual training of nurses
15. Ensuring compliance with applicable licensing as it pertains to health care
16. attendance at various departmental and committee meetings
17. Attendance when needed at SIS, ISP and team meetings
18. Engages in monthly clinical supervision with the Nurse Manager to review:
19. Quality of care provided
20. Charts of people with high-risk, complex medical conditions
21. Person-specific health risk protocols to determine if protocols are effective
22. Medically involved incidents (hospitalizations, medication errors)
23. Is able to perform all Essential Duties and Responsibilities of a Licensed Practical Nurse:
24. Prepares and administers medications, treatments and feedings as ordered by the physician. Assures accuracy in the right person, medication or treatment, dose, route, and time of administration. Assesses the effectiveness of the medication or treatment and observes for any advers
