Job Summary:
The Exercise Program Facilitator position provides a first impression for the program, it is imperative that this position consistently maintains a friendly, upbeat demeanor, coupled with the ability to motivate and convince individuals in Phase I of the importance for participation in the vital exercise intervention. This position is ultimately responsible for ensuring all program components are completed and tracked in a timely manner. Constant communication with program participants is imperative. In addition, this position will play a key role in assessing and developing performance improvement (PI) projects for HS, reporting monthly outcomes to both the Coordinator and Manger, providing updates to staff, and establishing and maintaining a good working relationship with respective physicians.
Education:
- Bachelors Exercise Science or Health Promotion Required
Certifications and Licensures:
- BCLS Required
- ACSM Exercise Specialist certification required or similar certification and must be completed within 12 months of employment.
Additional Knowledge, Skills, and Abilities:
- Good communication skills.
Job Specific Duties:
Program Orientation
Conducts orientation sessions; Ensures all necessary paperwork is secured, such as physician referrals, scholarship forms, etc., before patient begins program; Communicates by phone to each person entering Heart Strides (HS) to verify entry date and directions to Fitness Center for cardiac orientation or pulmonary consult.
Program Education
Provides Phase I education and program awareness to in-patient candidates; Participates in classes, monitoring participants, leading exercises, and providing educational sessions.
Progress Monitoring
Maintains a checklist to ensure that each participant in Heart Strides is completing all required components (e.g., six-minute walk tests, diet habit surveys, etc). Establishes goals with each participant and periodically monitor progress toward stated goals.
Performance Improvement
Assesses, develops and maintains Performance Improvement projects for the Heart Strides program. Reports specific project statistics to Coordinator and Director each month.
Work Relationships
Develops good working relationship with physicians and other pertinent healthcare professionals associated with Heart Strides.
