Medical Case Coordinator - Residential RN or LPN

The Medical Case Coordinator is responsible for the development, implementation and supervision of all systems involving medication administration, health and medical oversite, and training needs for the assigned program(s). Hours are primarily in the day however evening and weekend availability may be required based on the needs of the program. This includes assisting in overseeing the proficiency and delivery of medical related services for the assigned program. The Medical Case Coordinator will assist with the oversite of medically complex clients including but not limited to attending appointments, assisting with hospital admissions and discharges, coordinating care with the health care providers and medication and treatment administration as needed.

The Medical Case Coordinator is responsible for maintaining the training records for medical related trainings such as MAP, OSHA, First Aid, CPR and Unique Medical Needs of the clients. The Medical Case Coordinator works directly with the Director of Health Care Services in ensuring that all individuals receive appropriate support and care for all health care issues.

The Medical Case Coordinator will maintain all MAP regulatory requirements for the assigned program(s) though monitoring and medication audits and will work in conjunction with Quality Assurance (QA) to ensure that all MAP site locations are functioning under current State regulatory guidelines. The Medical Case Coordinator will assist QA during licensing reviews in the areas of MAP and Medical Training. This is accomplished by conducting reviews of medication administration practices at regular intervals that include but are not limited to:

a) Administering medications and treatments per LHCP orders

b) Observing Certified and Licensed staff administering medication to verify that they are following correct MAP policies, procedures and protocols

c) Verifying that the prescribing practitioner’s orders are on site, current and properly transcribed.

d) Verifying that medications dispensed by the pharmacy corresponds with those medications prescribed by the practitioner; verifying that all medication is being administered in accordance with the prescribing practitioner’s written order; verifying that medication is being properly procured and stored.

e) Maintaining EHR including, but not limited to, creating health encounter forms, APE packets, medication orders, uploading encounter forms, med orders, relevant health information, etc.

f) Updating HCSIS Health Care Record as needed.

g) Verifying that the program site has a current Massachusetts Controlled Substance Registration (MCSR) on-site and appropriate documentation of all Certified and Licensed staff administering medication at the site.

h) Reviewing documentation of medications being administered at the site to ensure that proper clinical protocols are in place and are being observed, e.g., monitoring vital signs, blood glucose monitoring, and appropriate practitioner notification; and,

i) Reviewing Medication Occurrence Reports (MORs) to determine areas where further training or other intervention may be needed

j) Coordinating and attending health and medical appointments, attending ISP/DDS/Stakeholder meeting for assigned individuals.

k) Ensuring nursing scheduling is complete and assisting with filling open shift.
