Responsibilities of a home-based Occupational Therapist include:

·         Provide high quality evaluation and treatment of neuromusculoskeletal disorders ·         Develop personalized care plans to improve mobility, strength, and overall function of patients in the home setting·         Communicate and collaborate with associated medical professionals to achieve patient goals·         Clear, concise documentation in our software system·         Regular communication with physicians, nurses, case managers within Veterans Affairs as needed·         Ability to work independently and autonomously, managing one’s own schedule·         Supervise home health aides involved in patient care
