Patient Navigator

WORK SCHEDULE: Full time, Monday - Friday, 9:00 a.m. - 5:00 p.m. (all days on-site)

SUMMARY: Provides support and follow-up to clients and outreach across all Care Coordination teams.

RESPONSIBILITIES:

•    Weekly chart reviews
•    Schedule medical appointments
•    Hospital visitations; Hospital/ER alert follow up (24-48 hours)
•    Initial assessments 60 days (newly enrolled members)
•    Client escort as needed
•    Scheduling transportation (Google Share Drive- MAS Transportation)
•    Fieldwork: Conducting Wellness Checks to follow up with clients; pantry delivery; documentation signatures; obtain paperwork as needed
•    Appointment reminders; follow up as needed
•    Medical Requests; follow up as needed (e-Fax OR in-person)
•    Daily submission of verification form(s) (tasks completed)
•    Healthix and PSYCKES database
•    Front desk coverage as needed
•    Flexibility based on needs of each team

•    High School Diploma/GED required.
•    Experience working with chronic illness, substance abuse, and/or mental health preferred.
•    Bilingual Spanish/English a plus.
•    Clean and valid driver's license with the ability to drive passenger van needed.
•    Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.
