Work Schedule:
Friday 5p-9p
Saturday/Sunday 8a-9p

Requirements
Functions:
- Successful execution of triage incoming calls with excellent customer service at all times. Remains prepared to demonstrate sound clinical judgement throughout shift. Takes all calls in quiet and confidential environment.
- Assigns visits as needed with good use of critical thinking and fairness for whom to assign to.
- Ensures all PRN visit needs are plotted consistent with agency plotting guide.
- Utilizes sound clinical skills and decision making to avoid sending visit nurses for unnecessary situations.
- Completes scheduled phone calls and charts outcomes in the medical record.
- Communicates with nurses and other members of after-hours team in a manner consistent with Paradigm’s culture expectations.
- Provides all pertinent information to visit nurses needed for visit nurses to be successful.
- Instills level of urgency with after-hours team with response time and follow-up.
- Demonstrates critical thinking to shift the plan as needed and prioritize one visit over another.
- Oversees after-hours strategy to remain fiscally responsible with appropriate use of Primary vs. Back-up nursing options.
- Fulfills the role of being a resource for new hires or other members of the after-hours team with questions, concerns, etc.
- When death visits are declined (rare), triage RN ensures death follow-up items are completed timely and thoroughly, including: notifying family, physician, funeral home, documentation, etc.
- Oversees difficult situations, grievances and staffing challenges with autonomy but involves applicable members of leadership if needed. Communicates concerns with appropriate leadership the next business day if not already informed. Communicates grievances and follow-up efforts per agency policy.
- Timely and accurate upkeep of call log with all pertinent information included.
- Complies with adding all pertinent notes into WellSky timely.
- Obtains and enters all verbal physician orders as indicated (e.g. with med changes, frequency changes, level of care changes, etc.).
- Submits all medication needs and refills timely through WellSky/Enclara when indicated. Coordinates mode of delivery of medications. Double checks medication orders to ensure successful transmission.
- Assists after-hours team with eligibility review and processing new referrals.
- Assists after-hours team with faxing out requested information (e.g. POST forms) if applicable.
- Assists after-hours team with execution of patient location changes or other occasional requests.
- Assists with referrals/admissions by reviewing paperwork, obtaining certification, pre-ordering meds, verifying DME to be obtained, etc.
Experience:
- Current RN Licensure with a minimum of 2 years of general nursing experience and at least 1 year of hospice nursing experience.
- Valid state driver’s license and reliable automobile.
- Proof of current automobile insurance.
- Current health certificate/physical examination and TB testing.
- Criminal background check.
Credentials:
- Licensed Registered Nurse in State of Indiana
