Job Description:

Duties/Responsibilities

  • Provides assessment, intervention, prevention, and education for wound care in the inpatient and ambulatory settings for patients within scope of organization

  • Develops and monitors proactive, individualized wound care plans using contemporary best practices, aimed at prevention and mitigation of wounds in support of patient’s overall care plan

  • Responsible for cleaning and bandaging patient wounds for a variety of surgical and/or long-term wounds

  • Collaborates across multidisciplinary care teams about wound treatments and any clinical changes to wounds to optimize patient care plan; provides regular follow-up to primary care teams related to necessary adjustments to a patient’s care plan

  • Maintains professional awareness of current wound-care research and techniques, and products used for wound care

  • Provides wound education and facilitates learning for patients and families

  • Accountable for meeting the standards of patient/personal safety and in maintaining a clean and therapeutic environment

  • Serves as a preceptor to other nurses or students

  • Maintains patient information and documentation in electronic medical record

  • Abides by organizational and HIPAA guidelines, privacy practices, patient confidentiality, and patient rights

  • Supports hospital and departmental performance improvement goals

  • Provides quality care through compliance with departmental and organizational policies and guidelines, and external agency guidelines, standards, or regulations

  • Supports departmental initiatives and change efforts

  • Practices financial and resource stewardship

  • Engages in strong communication, acts as team player in supporting mission and vision of organization, and abides by organization’s core values

Required Skills/Abilities

  • Graduate of accredited school of nursing

  • Bachelor of Science in Nursing (BSN) required

  • Current license to practice professional nursing in the state of Texas

  • American Heart Association BLS certification required

  • American Heart Association PALS certification required, or within 6 months of hire

  • Minimum 4-years of relevant nursing experience required

  • Minimum 2-years of wound-care experience required

  • Certified Wound Care Nurse (CWCN) or Certified Wound Ostomy Continence Nurse (CWOCN) credentials or equivalent preferred

  • Physical Requirements: Lifting/Carrying, Pushing/Pulling and Repetitive Movements

  • Lifting or carrying between 75 - 100 lbs.

  • Sensory/Communicative activities essential to the performance of this position: Hearing, Seeing, and Speaking

  • This position will be exposed to Inside Environments, Bloodborne Pathogens and Chemical Hazards
