Responsibilities

The Targeted Case Manager provides comprehensive case management services to eligible individuals to support access to needed medical, behavioral health, social, educational, and community-based services. This role is responsible for assessment, care planning, service coordination, monitoring, and advocacy to ensure services are appropriate, timely, and responsive to the individual’s evolving needs.

 

Essential Duties & Responsibilities:

 

  • Conduct comprehensive assessments and ongoing reassessments to identify medical, behavioral health, social, educational, and service needs.

  • Develop, implement, and update individualized care plans in collaboration with individuals, families, and authorized supports.

  • Manage an assigned caseload in compliance with state regulations, payer requirements, and program standards.

  • Coordinate referrals, appointments, and access to services across providers and community resources.

  • Monitor service delivery to ensure care plans are implemented effectively and remain appropriate to individual needs.

  • Maintain regular, required contact with individuals to support continuity and quality of care.

  • Collaborate with individuals, families, caregivers, providers, and stakeholders to ensure coordinated service delivery.

  • Identify changes in needs or circumstances and make timely adjustments to care plans and services.

  • Ensure accurate, timely, and compliant documentation in accordance with regulatory and organizational standards.

  • Maintain full compliance with all applicable payer, state, and federal requirements for targeted case management services.

Qualifications

Education & Experience:

  • State Specific Requirements

    • Mississippi:
    • An individual who holds at least a Master’s degree in an addictions, mental health, intellectual/developmental disabilities, or human services / behavioral health–related field and meets one of the following criteria:
    • Holds an applicable professional license; or
    • Holds a DMH credential as a Mental Health Therapist, Intellectual and Developmental Disabilities Therapist, or Addictions Therapist, as appropriate to the service and population being served.
