About the Role

As a Physical Therapist at Harbor Haven, you’ll deliver patient-centered care that empowers residents to regain independence, enhance mobility, and enjoy a higher quality of life. You’ll work closely with a multidisciplinary team to create meaningful, individualized treatment plans that support each resident’s goals.

Key Responsibilities

• Perform initial and ongoing assessments of residents’ strength, mobility, balance, and endurance

• Develop and implement individualized care plans that address physical barriers to independence

• Use targeted therapy interventions to support recovery, mobility, and daily function

• Educate residents and staff on safety practices and fall prevention strategies

• Recommend and train in the use of assistive devices

• Collaborate with nursing, occupational, and speech therapy teams for comprehensive care

• Maintain accurate clinical documentation to meet compliance standards
