Care Manager (RN) - In-Patient Medicine - Days

The Case Manager on the Medicine Service Line is a critical member of the Medicine Service Team who aids in the care, development, education and support of all medicine patients and families. The Case Manager plays a key role in patient throughput, patient escalations and length of stay during a vulnerable time in the lives of patients and families. The Case Manager is responsible for completing assessments, PRI, educating, and supporting the patient and family in identifying the best discharge plan that will lead to a healthy and more productive life. The goal is to provide the highest quality care and transition of care for patients from the acute setting to the next level of care. The Case Manager plays a key role in the process of developing comprehensive discharge plans, beginning on the day of admission.

Preferred Criteria

  • 2 years' of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting 

  • 1 year of leadership experience in a healthcare setting

  • Master's Degree in healthcare related field 
     

Required Criteria

  • Bachelors degree in Nursing (BSN)

  • Current NYS Registered Nurse (RN) licensure

  • 3 to 5 years of health care experience in an acute care setting

  • Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
