Job Summary:
This is an opportunity for an Advanced Practice Provider (APP) to see chronic kidney disease and dialysis patients in an outpatient setting.
Key Responsibilities:
· The APP will be responsible for the evaluation and management of patients with chronic kidney disease requiring hemodialysis and work with physicians in determining best management.
· Oversee management of patients on maintenance hemodialysis.
· Perform history and physical examinations
· Review diagnostic testing,
· Check medication dosages and adjusting medications,
· Check laboratory values
· General assessment and provide educational guidance to patients with advanced chronic kidney disease and requiring hemodialysis
Work Location:
Onsite – this role is expected to work onsite and is located in Aurora, Colorado
