DEPARTMENTAL SUMMARY: St. Charles Population Health department serves as the core design and facilitation team for health system population health and management services. The Population Health department is integrally linked to payer contracting, care coordination in the inpatient and outpatient setting, local and regional health plans, community needs and integration, public health and regional non-profit entities, and data analytics.

POSITION OVERVIEW:

The Ambulatory Care Pharmacist is responsible for medication management of patients in the ambulatory setting through a collaborative model that includes clinic providers, staff, resource specialists and clinical resource coordinators.  This includes disease state management, patient education, optimization of drug therapy regimens, and other initiatives designed in collaboration with the clinic providers and staff.

The Ambulatory Care Pharmacist will serve one or more of the Family Care Clinics.  

This position does not directly manage any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES:

Manage patients’ chronic disease states via collaborative drug therapy management (CDTM) in conjunction with primary care providers and extended care team. CDTMs include, but are not limited to the following conditions, diabetes, asthma /COPD, depression, hypertension, anticoagulation, medication adherence and other chronic diseases.

Actively participates in drug reconciliation and transitions of care pharmacy management including transfers between home/long term care, acute inpatient care, and other sites as requested.

Provides telephonic and in-person disease state management services to include complex/polypharmacy medication review, drug-drug interaction checking, patient education and interventions to improve medication therapy adherence.

Conducts comprehensive and targeted medication review for pre-defined populations and in accordance with contractual requirements.

In collaboration with primary care provider, ensures optimal patient care outcomes from drug therapy.  Ensures services are age appropriate.

Participates in the development, implementation and measurement of outcomes of disease state management initiatives in the ambulatory setting.

Works as part of a multidisciplinary team to provide patient-centered care as part of the Primary Care Medical Home in conjunction with RN Care Coordinators, Community Health Specialists, Behavioral Health Consultants, Medical Assistants, Primary Care and Internal Medicine providers.

Assures appropriate documentation of drug therapy interventions in patient’s medical record.

Provides ongoing education on medications to patients, providers and clinical staff.

Provides timely interventions to optimize patient flow within the clinic setting.

Maintains strong rapport with health care practitioners.

  

Participates in development of patient education materials particularly with regard to medication and adherence topics.

Develops and maintains knowledge of federal and state regulations, basic insurance knowledge, benefits, services and limitations.

  

Assists in the planning and implementation of group patient visits and disease management, where appropriate.

Provides support to meet reporting requirements of organization’s contractual obligations in terms of pharmacy compliance and management.

Participates in creating a healing environment that supports all aspects of care and the wholeness of each individual, patient and caregiver.

Documents all patient care with proficiency in compliance with hospital policies and regulatory agencies.

   

Delivers customer service and/or patient care in a manner that promotes goodwill, is timely, efficient and accurate
