- Tech Savvy: You must be able to explain mobile app technology to a non-tech savvy, elder population, as well as troubleshoot mobile phone and app issues.
- Comfortable with G-Suite: You must be comfortable with working with our internal messaging (Google Hangout, Zoom, Google spreadsheet, pdf, printers, etc. ).
- Sensitive and patient: our primary patient population is elderly. They may be forgetful, hard of hearing, or have physical difficulties and limitations requiring patience, adapting your teaching style, and repetition.
- Have a Growth Mindset: As we grow, our protocol, workflows, and scope of care grows with it. We look for teammates that can accept feedback and adapt; and teams that can learn, implement, and offer constructive feedback to improve processes for the greater good.
- Bilingual a plus especially Spanish, Cantonese, Mandarin, Tagalog, Vietnamese, etc.
License/Certifications Required
RD/RDN (Registered Dietitian/Nutritionist)
Primary Duties and Responsibilities:
- Help set up and teach our elderly patients about how our mobile app and medical devices work, and help them troubleshoot any tech issues.
- Collaboratively develop an individualized care plan, behavior goals and learning objectives with each patient. Provide individual case management to patients and keep documents with HIPAA compliance.
- Use the established Standards of Practice and Professional Performance (SOP/SOPP) in diabetes care and evidence-based practice guidelines to determine the educational interventions for each individual.
- Collaborate with primary care physicians and other clinicians
- Utilize our iHealth Unified Care software and incoming vitals from our devices to track and monitor outcomes and provide routine goal setting and educational interventions.
- Support patients in our program through our online platform during daily online shifts and during visits.
- Work with a cross-functional product team to develop and constantly improve digital health solutions to extend the chronic disease management beyond the doctor's office.
- May have an opportunity to mentor others within our clinical service team, provide input on clinical protocols for improvement, or collaborate on internal research studies.
- Depending on skillset, portfolio, and desire, create educational blog articles, videos, and/or handouts for our chronic disease self-management education program that meets AADE and AHA standards.
- Other duties and projects as assigned.
