Overview
LifeLong Medical Care is searching for a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT),or Licensed Professional Clinical Counselor (LPCC) -or- Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), or Associate Professional Clinical Counselor (APCC) to join the team at our William Jenkins Health Center. Under the general supervision of the Associate Medical Director and Psychosocial Services Manager, the LCSW/LMFT/LPCC or ASW/AMFT/APCC will work alongside an interdisciplinary team to ensure that our pediatric patients aged five and up receive the highest quality psychosocial services possible. And you will have the opportunity to make a difference in the lives of our patients and their families.
The ideal candidate will have experience providing psychosocial services in a medical setting. You will be passionate about working with pediatric patients and dedicated to providing compassionate care to all of our patients.

This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA.

We provide medical, dental, behavioral health, and school-based services at more than 15 primary care sites across Oakland, Berkeley, and West Contra Costa County. Benefits:
We offer a competitive annual salary of $68,000-$72,000 for ASW/AMFT/APCC, $100,000-$120,000 for LCSW/LMFT/LPCC, depending on years since licensed, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs (HPSA score is 16). Acceptance rates may vary year to year based on funding levels and the number of applicants.
Responsibilities
- Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and implements short-term counseling using appropriate modalities for patients with issues such as: depression, anxiety, trauma, grief/loss, addiction, domestic violence, behavioral health problems, and psychosocial stressors. Works predominantly within clinic, but may include home visit services.
- Provides short-term case management services for patients participating in treatment.
- As a team member, assists with crisis intervention and triage as needed. Crisis intervention includes assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
- Works with team members to provide follow-up assessment and prevention planning after any crisis, including follow-up with outside agencies.
- Maintains clinical and administrative records in accordance with contractual and billing requirements and submits encounter forms on a weekly basis.
- Participates in weekly clinical meetings; promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
- Participates in quality improvement efforts related to behavioral health and integrated models of care.
- Consults informally with providers as needed.
- Follows all State of California mandated reporting requirements.
- May serve as supervisor for psychology or MSW interns and may provide ASW, AMFT or APCC clinical supervision.
Qualifications
- Ability to work effectively and collaboratively in a fast-paced primary health care setting and prioritize completing job demands.
- Must be a self-starter, able to balance counseling, case management, and administrative tasks.
- Demonstrated ability and sensitivity to working with a diver
