What You’ll Do
• Work under the direction of the Physical Therapist to implement individualized care plans for residents.
• Provide hands-on therapy services to restore function, improve mobility, and promote overall wellness.
• Accurately document resident progress and communicate effectively with the therapy and nursing teams.
• Participate in care plan meetings and interdisciplinary collaboration to ensure quality outcomes.
