Bilingual Case Management Coordinator - Field - Must reside in Tampa Florida

Position Summary

This Bilingual Case Management Coordinator position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based position out of Hillsborough County and surrounding areas in FL. The requirements is for candidates to live in Tampa Florida, and to travel 50-75% of the time to meet with members face to face.

We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team.

Case Management Coordinator is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Case Management Coordinator will
effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Case Management Coordinators will determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.

• Candidates must be located in Tampa, FL
• Candidate must also be fluent bilingual in Spanish (reading, writing, and speaking).
• Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough county and surrounding areas in FL
• Schedule is Monday-Friday, 8:00am-5:00pm, standard business hours

Required Qualifications

• Bi-lingual - Fluent English and Spanish (reading, writing, and speaking)
• Must reside in Tampa, FL
• Must possess reliable transportation and be willing and able to travel up to 50-75% of the time o meet members face to face in Hillsborough county and surrounding areas in FL. Mileage is reimbursed per our company expense reimbursement policy
• 1 + year of experience in behavioral health or a long-term care setting
• Effective communication skills, both verbal and written
• Computer proficiency in Microsoft Word, Excel, and Outlook
• Schedule is Monday-Friday, 8:00am-5:00pm, standard business hours

Preferred Qualifications

• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Managed care experience
• Case management and discharge planning experience

Education

• Bachelor's Degree in Social Work or related field
