This role is ideal for someone who thrives on human connection, understands the importance of health education and wants to make a meaningful difference in the lives of people with chronic kidney disease.
As a key player in a new pilot program, you will serve as the face of our initiative in your local market—spending time inside dialysis clinics, meeting kidney care patients where they are and building relationships that lead to better understanding and participation in our program. You’ll also play a critical role in capturing insights, testing new ideas and helping us scale what works.
Key Responsibilities:
Serve as the local presence for our value-based care program, engaging directly with kidney care patients, caregivers and community stakeholders to share information and answer questions
Develop strong, collaborative relationships with in-clinic teammates, social workers, nurses and physicians to support patient engagement
Immerse yourself in the program’s offerings and eligibility requirements to communicate effectively while upholding regulatory and compliance guidelines
Identify barriers to engagement that bring forward real-world insights and creative ideas to improve outreach efforts
Document key learnings and help codify outreach best practices, talking points, workflows and community engagement strategies
Track basic metrics to support evaluation of pilot effectiveness and inform scaling decisions
Potential to represent the program at local community events, health fairs and grassroots opportunities to raise awareness and build trust
Travel to dialysis clinics four days per week throughout the Atlanta metro area
Occasional availability to work on Saturdays and during events, if desired
Qualifications:
High school diploma or equivalent is required
2–4 years of experience in community outreach, patient advocacy, healthcare engagement, grassroots marketing or related fields is preferred
Background in healthcare, nonprofit, education or public health a plus
Strong interpersonal and communication skills—you enjoy talking to people from all walks of life and building relationships
Deep commitment to ethical communication and regulatory compliance
Comfortable working independently and self-managing priorities in a field-based role
Organized, adaptable and energized by a fast-paced pilot environment
Proficiency with Microsoft Office and digital collaboration tools
Must reside near the Atlanta metro area, possess own, reliable transportation and a valid Georgia driver’s license—frequent local travel required as most time is spent inside clinics. This role will drive their personal automobile to get from one clinic to another.
Bilingual / Spanish speaking is strongly preferred
